Camp Registration
Register today for one of our Hodia camps!  Please read our privacy statement if you have any questions about the information you are submitting. Our registration is done through a secure network. Our registration is done entirely online. There are no papers to fill out—and all consents for camp attendance and activities is part of our online registration. Current open camp registrations can be found on the grey panel to the left under "Hodia Online Registration".

This form is needed only if you are applying for scholarship funds through the ADA (American Diabetes Association)

Must be completed each year by all staff members that will be driving at camp.

Staff Forms - Reimbursement
Note: If you are buying goods or services on behalf of IDYP (requires the permission of a board member), you may seek reimbursement for those expenses by completing a reimbursement form that you may download below. When making a purchase for IDYP, please turn in a complete Sales Tax Exemption form (also available for download below) to the vendor/seller and avoid paying state sales tax.
